What else do I need besides “110% of Medicare”?

Identify the reference schedule, setting, locality and period before applying a Medicare percentage.

By DueMDEditorial answer

No. A percentage tells you how to scale a reference amount. It does not identify the complete reference or establish which rules apply.

First identify the amount being multiplied

If the relevant reference amount were $100, multiplying by 110% would give $110. That is an arithmetic example, not a current Medicare rate or the expected payment for a service.

Find the reference first

Use the agreement and supporting records to establish the fee schedule, version, setting, and other inputs that the terms require. Check the effective period rather than assuming that the most recently published file is the one your agreement uses.

Use the Medicare reference tool for its supported dataset and calculation limits. CMS’s Physician Fee Schedule reference files are the source to consult when establishing the appropriate reference. An article’s worked example is not a live rate.

What to enter in the contract calculator

The primary and second-procedure references are amounts you supply. The model then applies the multipliers you select. It will not tell you whether you chose the right reference, whether a service is covered, or whether a particular exception applies.

For a rate comparison, keep a record of how you arrived at each input and have that reviewed separately. To explore the mechanics, start with the worked calculator example. Do not paste private fee schedules into a question.
