START WITH AN EXAMPLE

Choose a rate change.

Choose a rate-change example. The generator uses those inputs to create an editable review checklist—not a report that testing has already passed.

A percentage changes

100% becomes 108%. Check the date boundary, basic arithmetic and an excluded product.

A second service is reduced

Keep the first eligible whole-service amount at 100%; use 50% for the second.

A fixed fee changes

Compare $100 and $108 per defined unit, with a separate check for excluded entities.

These examples illustrate the templates. Edit the inputs to build your checklist; no document upload is needed.

1 / DESCRIBE THE CHANGE

What is changing?

Use public or illustrative terms only. Do not enter confidential contract information.

The worksheet assumes the service date is the trigger. Confirm that in the actual terms.
Percentage of reference
Example: 108 means 108%, not an extra 108%.
Blank means the previous rule needs review.
Add review dimensions

Five core checks are always included: before, on and after the date, a basic calculation, and rounding/units.

How to use the checklist

Have a qualified reviewer verify the expected conditions. Run the tests in an authorized process, record actual results, and retain the evidence. A listed test with “review required” is an open question, not a failure or a pass.

The examples use illustrative reference amounts to make arithmetic visible. They are not Medicare rates. Modifier effects and alternative product terms are not inferred.

Where the template stops

This is a template-based work plan. Document interpretation, payer-system testing and approval remain outside its scope.
